The Clerk's Office has two different payment history reports available-an
Official Payment History and a Summary Payment History.
The Official Payment History is a detailed report containing several
pages that is often used by the Court and other entities requiring
a certified document. This is a complete history from the first
payment forward.
To obtain an Official Payment History, you may either come to either
of these Clerk's Office in person at 201 W. Jefferson St. in Phoenix,
222 E. Javelina Dr. in Mesa, 18380 N. 40th St., Suite 120 in Phoenix, 14264 W. Tierra Buena Lane
in Surprise or write to:

When visiting the office in person, the fee for an Official Payment
History is $18.00. For written requests the fee for a mailed certified
copy of your payment history is $23.00 (which includes $5.00 for
postage and handling).
The Summary Payment History is available through the FAX Back System.
A summary of payments listing the monthly amounts paid during the
current year and the previous year is transmitted via a fax machine.
This summary is a short version of the Official Payment History
and will not be certified.
There is no cost for you to receive this report through the FAX
Back System.
