New Employer/Change of Employer

How do I change the employer information with the court?
You will need to provide in your name, court case number, ATLAS number, obligor's name, the employer's name and payroll address. The Current Employer Information form is available by clicking here. This form must be completed for notification of a change of employer. Simply fill in the requested fields and click "Send" to submit the informtion to the Maricopa County Clerk of Superior Court's Office, Family Support Finance.


How do I get the Order of Assignment mailed to the new employer?
If this is a private case, (Non IV-D), you will need to provide, in writing, the court case number, ATLAS number, obligor's name, and the employer's name and payroll address. Date and sign the written notice and FAX to (602) 506-1937 or mail to Clerk of Superior Court, Family Support Finance, 201 W. Jefferson, 1st Floor, Phoenix, AZ 85003.

If this is a IV-D case, call DCSE at (602) 252-4045 or provide in writing the ATLAS number, obligor's name, Social Security number, employer's address, date and sign the written notice and mail to DCSE, P.O. Box 40458, Phoenix, AZ 85067.
